Deceased Donor Process: Specially trained transplant coordinators provide grief counselling and approach the next of kin for consent, documented using standardized NOTTO-approved forms under THOTA. Potential donors are identified in critical care settings, and brain-stem death is certified by a qualified medical board. Organs are then allocated through the NOTTO/ROTTO/SOTTO network based on medical urgency, blood group compatibility, and wait-list status — with no upper age limit or domicile restriction for recipients.
Living Donor Process: A Near Relative (spouse, children, parents, siblings, grandparents, grandchildren) is evaluated by a Competent Authority comprising a Medical Superintendent, four doctors outside the transplant team, and two independent members. A donor who is not a near relative is instead evaluated by a Hospital-Based Authorization Committee comprising the hospital head, two non-transplant doctors, two non-hospital members, and two government representatives. If an application is rejected, the family may appeal to the Appellate Committee.
